On Tuesday !9 March 1991, you were interviewed by a Television Crew
at the Sto~ffville Fire Hall. You were off-duty but in full Uniform
at the time. The interview at the Station had been approved
vez'bal].y by the Operator, Mr. P. Carell, that morning.
While 5he interview was in progress, the Duty Crew received a code
four call[ to a bJursing Home.

i) into the back of the Ambulance,
ii) into the Nursing Home and Patient area
iii)into the Hospital
iv} To record ~ events as they occurred,
~- Attend to the Patient al%hough Off-Duty,
You did not, however;
A - Obtain permission f~om Management for an~ of the above points,
B- Obtain a signed Waiver or Release from: Liability f~om the
Cameraman or the Television Station,
C- Obtain a signed Waiver or Release from Liability from the
Patient or the Nursing Home, ~,
D - Follow the Policy & Procedures Manual ~n ~y respect.~ ~

It should be noted that as these activities were unauthorized, you
were not protected by any of the variot, s Insurance Policies ~.:e

Pat iel~t/Guardians for Invasion of Privacy and related matters.
