ORDER
Further to the submissions made by counsel, and consent of the parties, I hereby
order the production of the following documents
1 Name and contact Information for the Individual who conducted a de-
bnefing on Thursday, Apnl 6, 2000 regarding the Incident of Apnl 4, 2000
2 A copy of the Investigation Report as well as all documentation reviewed
and!or considered! and or attached to the Investigation Report carned out
by Mr David Andrusko of the Thunder Bay Correctional Centre
3 All occurrence reports filed for the Youth FacIlity from March and Apnl
2000
4 A copy of the log book for the facIlity for Apnl 3rd through 30th, 2000
5 A copy of the resident files for the female young offenders allegedly
Involved In the Incident of Apnl 4, 2000
6 A copy of the personnel file, and In particular the documentation pertaining
to the training of Mr David Hrbowy
7 The Apnl 4th, 2000 check-off sheet for resident's cleaning duties for the
Young Offender units of the Thunder Bay Correctional Centre
8 A copy of the note found In the possession of the residents by staff
documenting the planning of the attack on the staff
9 A copy of the Internal Investigation Union report, together with all notes of
the Investigator and all documentation considered by or Included or
referred to In the report by the Investigator
10 A copy of the directives given to the Investigator In the conduct of the
Investigation
11 If not already Included In the above, copies of occurrence reports from all
other staff for the Incident on Apnl 4, 2000
12 Name and contact number of the Ministry de-bnefers who met with the staff
approximately two weeks following the Incident, together with such policies
and directives that are applicable to the de-bnefing

13 A copy of a letter sent on or about the Fall of 1999 directed to Mr John
Horner from staff at the Thunder Bay Correctional Centre regarding staff
concerns
14 Schematic/layout diagram of the Youth FacIlity of the Thunder Bay
Correctional Centre
15 Documentation regarding the Health and Safety Investigation and order
conducted by the Ministry of Labour on or about Apn111, 2000
16 Information regarding the Employer's compliance with the same
17 A copy of the Ministry and Institutional standing orders for 1999
18 A copy of the personnel file of the gnevor, Anna Gibbon, Including such
WSIB documentation as the Ministry has In Its possession
19 Notes or correspondence between management and the Police and/or the
Courts concerning the charges laid against the young persons as a result of
the Incident of Apnl 4, 2000
I also order the parties not to disclose the names of any of the young offenders
that may be or allegedly be Involved, and that the Information be used only for the
purposes of this heanng, after which any documentation Involving the young offenders
shall be destroyed
